Flu Vaccine Questionnaire and Consent
Cold and flu season is right around the corner.
Niobrara Valley Clinic is offering a flu shot clinic at each campus in the coming weeks for both staff and students.
Spencer: Monday, October 26th
Lynch: Tuesday, November 3rd
Butte: Monday, November 9th
from 7:00am-8:15am all three days.
If you are interested, a flier as well as other paperwork to fill out is attached.
